I am really enjoying this form maker. I like the versatility and being able to do more complex forms without needed special plugins for WordPress or having to do tons of hand coding. I would recommend you get this. It is one of those tools you may not use all the time but is perfect when you do need it.

Clean, elegant and fantastic support. Form Engineer is fantastic. It's straightforward and simple to use, but has all the bells and whistles you might need for forms or surveys including radio buttons, check boxes etc. It also has upload file and signature functionality which means I can eventually move my contract-signing over and save some moola.
